Kelly Pappa

Dedicated, patient-centered registered nurse with over 30 years of experience in case management and care coordination across inpatient, outpatient, and community settings. A proud graduate of St. Francis Hospital School of Nursing, Kelly is passionate about guiding patients and families through the complexities of the healthcare system—ensuring they receive timely, appropriate, and compassionate care.

Kelly has led transformative initiatives across the healthcare continuum, including spearheading the Northwest Transitions in Care Community (NWTCC)—a collaborative of over 30 organizations focused on reducing hospital readmissions and improving care transitions. She also launched a community-wide Congestive Heart Failure (CHF) program that supported patients and caregivers in managing chronic disease and improving daily quality of life.

As Clinical Director at the Connecticut State Medical Society IPA, Kelly worked closely with independent providers to strengthen their performance in value-based care models. She later served as a clinical executive at Cigna, driving population health strategies and collaborating with large employers to improve outcomes and reduce costs.

Now returning to her life’s purpose—helping others thrive—Kelly is focused on improving the quality of life for patients and providing peace of mind for their caregivers. Committed to advocacy, compassion, and sustainable wellness, she empowers individuals and families to make informed decisions and feel supported every step of the way.
